    Provide the following onboarding requirements:

    1. Filled-out and signed Business Application and Information Sheet
    2. Photocopy of DTI/CDA/SEC Registration
    3. Photocopy of updated Mayor's Permit
    4. Photocopy of BIR 2303
    5. Secretary's Certificate of Authorized Representative (for cooperatives and corporations)
    6. One (1) government-issued ID of owner or authorized representative
    7. Three (3) specimen signatures of owner or authorized representative
