How to sign-up as a U merchant partner

    Go to any U Store branch.

    Fill up the Business Application and Information Sheet.

    1. Business Details
    2. Contact Information
    3. Bank Details

    Provide the following onboarding requirements:

    1. Filled-out and signed Business Application and Information Sheet
    2. Photocopy of DTI/CDA/SEC Registration
    3. Photocopy of updated Mayor's Permit
    4. Photocopy of BIR 2303
    5. Secretary's Certificate of Authorized Representative (for cooperatives and corporations)
    6. One (1) government-issued ID of owner or authorized representative
    7. Three (3) specimen signatures of owner or authorized representative
    8. 2x2 ID pictures of owner or authorized representative
    9. Bank Certificate
    10. MOA and NDA
